Preferred referral information

To help us plan the first consultation efficiently, please include the following where available:

• Patient demographic details and medical aid information
• Presenting problem and relevant clinical history
• Imaging reports and imaging access (disc, viewer link or referral)
• Pathology reports and receptor status where applicable
• Current medications and comorbidities
• Preferred contact details for the referring practitioner

How to refer

Communication with referrers

Referring practitioners receive a written summary after the initial consultation and at treatment milestones. Where multidisciplinary planning is required, we will notify you of the agreed plan before treatment starts. For urgent clinical questions between letters, the practice cell number is the fastest route.
